Report on Activities EAF See & Treat - FcP East Java Region Period 2011


FOREWORD

Dear all.

On behalf of See & Treat East Java Region, allow me to thank God that finally we can finish our report of activities 2011.

Although we just got the grant only for awareness as the first pillar but we are happy that we can develop the project to 4 districts consist of 3 regencies and 1 municipality. The training were 2 days for GP and midwives and 1 day for Public Health Nurses, Field Coordinators, and Teachers, while the content was not only for awareness but also the skill in screening and treatment, because we assumed and directed to the districts that after training they have to plan to buy the cryo unit and formed the local political will in the regulation of See and Treat. We promised them that we will be available for the next, follow up training conducted by themselves using their own budget.
Our experience in 2011 we found the high enthusiasm of the people and local government to this project as a demand. Therefore we just wondering that this beautiful program is still needed  to be prolonged for the rest of 22 districts in East Java Region, because until now cancer has not been as priority yet. During evaluation meeting we found that not all of the prior See and Treat districts are survive because of political policy which has not been established due to individual sight.

On this occasion I want to thank to the Dean of Medical Faculty Airlangga University and Director of dr. Soetomo Hospital who always support our activities. To the See and Treat team for the high  spirit to share the knowledge and skill in beautiful atmosphere and build such a solid teamwork.

To the Governments of Probolinggo Municipality, Ponorogo Regency, Probolinggo Regency, and Magetan Regency who wanted to be involved in this project and hopefully these activities could be developed in these districts and East Java Province in General.

Proposal See & Treat East Java Region

 
Background

            Cervical cancer is one of the major women problems, especially in developing country such as  Indonesia. Some data from some hospitals in Indonesia showed that the incidence of  cervical cancer is highest among  any other  cancer  in  women.  Data from 12 pathological
Center in Indonesia showed that the persentage of cervical cancer in women was 26,4 % and became  the  first  rank  in term of the most commons cancer in women.  Besides the high incidence, the other problem of cervical cancer is that mostly  cervical cancer patient came  in  the late stage, usually  in  the  stage  II B  and  beyond.  As we know, that the  cure  rate will be significantly  decreased  by  such  this  stage.

            In  order to  find  the  earlier  cancer  patients  and  to avoid patient with cervical cancer come  to  the  doctor  in  the  late  stage,  it  is important  to  develop the  screening  method  of which  will  effectively  find  the cervical  precancerous lesions,  the patients can  find easily to the Provider – Health Service Unit  and  not expensive  for the social-economic  patients.
First of all,  we have Pap smear  method  for  screening.  It  has  been  proved  to  decrease  the  incidence  of  invasive  cervical  cancer  from  90 %  decreased  the mortality  rate to 60-80 %. This Pap Smear Test was successful  as  the  effective  method  up  till  now.
The  other  Screening  method  to  prevent  Cervical  precancerous  lesions is  VIA  ( Visual  Inspection  by  Acetic Acid ).

            “See & Treat “  Project,  A  Collaboration  Project  between  The School of Medicine Airlangga University  and  Dr. Soetomo General Hospital – of which handled  by CBCC -  Indonesia  and  the  Female Cancer Programme  of  The  Leiden University Medical Center,  is  the  Brand  new  program  to  Avoid  Women Cancer, especially  for  Precancerous  lesion.
This  See & Treat  project,  uses  VIA  as the effective  screening for those.  If  the  precanceous  lesions  are  suspected,  it  will  be  treated directly  by using  Cryotherapy. 
If  the  cancer  is  suspected, so  the  patient  will  be  referred to the hospital.       

            The Objective  of  See &  Treat  Project  is  to  develop  the  sustainable  See & Treat
screening  service   - Single approach -  as  an  effort  to  improve The  Reproduction  Health of   the  Low  Economic Women  in  East  Java  Province.
            This Collaboration  project  will  be  executed  2007 - 2010  in  7  Regions  of  Indonesia.  Especially  in  East  Java  Province  will  be  held  in  January  2008 – 2010.
The  Executor  will  be The  Staffs  of  The  Department  of  OBSGYN, The School of Medicine of Airlangga University (SMUA) / Dr. Soetomo Hospital  and  CBCC  East Java  Province’s  staffs  for  the  awareness,  The Staffs of  East Java Province Health Service  Office  and  the  Cadres  of  PKK  of  some  Regencies and  Municipality. 
They  will  work   in  the Clino-mobile  and  or  Puskesmas of the  selected  areas.  The  Project   funded  by  The Ministry   of  Foreign Service  (MFS)  of  The  Dutch  Government.
